Does protein intake correlate with tubular function in very preterm neonates?

چکیده
Background High protein intake in very preterm neonates (VPN) is important for growth. However, kidneys have fewer functional nephrons and many of the ones present may be immature. Studies shown that high induces nephron hypertrophy, proteinuria, glomerular sclerosis, which lead to tubular injury. Urinary neutrophil gelatinase-associated lipocalin (uNGAL) a biomarker released during proximal cell The uNGAL creatinine (uNGAL/Cr) ratio commonly performed normalization.
Objective To assess possible association between uNGAL/Cr VPN.
Methods A prospective cohort study was conducted two NICUs Jakarta. Subjects’ urine specimens were collected at 0-48 hours, 72 21 days after birth determine as Protein administered according sites NICU guidelines. recorded daily from 14-21 age formula measured twice with human milk analyzer breast milk. ELISA used measure concentration. Low defined <3g/kg/day ?3g/kg/day. Maternal perinatal variables medical records.
 Results Fifty-nine VPN recruited, whom 39 completed study. Median ranged 0.32-104.11 ng/mg. not correlated but inversely gestational weight [r = -0.320, P=0.019 72-hr (T2) urinary collection]. Higher levels associated maternal infection [14.4 (range 4.4-104.1) vs 7.2 0.5–32.4) ng/mg, P=0.004 0-48-hr (T1)], anemia [6.9 1.2–66.6) 1.7 0.3–89.2) P=0.001 21-day (T3)] nephrotoxic medication [15.9 1.3–63.8) 1.0 0.4–8.6) P=0.026 72-hr].
Conclusion current nutritional guidelines does correlate injury VPN, by ratio. infection, anemia, lower weight, medication, higher VPN.

Immune status in very preterm neonates.
OBJECTIVES Preterm neonates are at increased risk of sepsis compared with those born at term. We investigated immune status at birth and early neonatal life in very preterm neonates and its association with short-term outcomes. METHODS Prospective observational study conducted at a university hospital recruiting 113 preterm neonates (23-32 weeks) and 78 controls. Monocyte major histocompatibi...

متن کاملAbnormal FcRIII expression by neutrophils from very preterm neonates.
To further investigate the neutrophil dysfunction of newborn infants, we have measured expression of the neutrophil Fc gamma receptors FcRIII and FcRII in extremely immature preterm neonates born at 24 to 32 weeks of gestation. Fc receptor expression was measured by FACS analysis of cells stained with monoclonal antibody Leu11b for FcRIII and IV-3 for FcRII. "Well" preterm neonates displayed re...
